While you are a patient at HSHS St. Joseph's Hospital, your primary focus should be your health. We realize you may also be concerned about your continued care needs following your hospital discharge.
Social Services can facilitate a variety of programs to help in-patients or out-patients and their families address the emotional and social problems that often accompany an illness or injury. Some primary functions of Social Services include:
- Guidance and support in coping with stress resulting from a difficult medical diagnosis or treatment
- Researching answers to a wide variety of questions troubling a patient and family
- Informing patients and families of existing resources and also providing information and assistance with completion of an advance directive such as a Power of Attorney for Health Care
Reach for Recovery/Navigator Program is co-sponsored by the American Cancer Society and coordinated through Social Services. This program provides contact with the American Cancer Society Navigator who will connect you with support programs or find information you want about cancer and cancer treatment. A Social Worker will meet with you to provide information about the Reach for Recovery/Navigator Program.
Telephone Service for the Hearing Impaired is available for anyone with access to a TDD. A TDD is a telephone service for the hearing impaired that uses a telecommunications device. Use your TDD to take advantage of this service and reach the Emergency Center 24 hours a day.
Power of Attorney
A durable power of attorney for health care is a legal document which nominates your partner or another person as your "agent" to act for you in making most health care decisions if you are unable to do so for yourself. This document can also contain instructions to your agent regarding your desires. It can state which types of treatments you wish to have provided, and which types you wish to have withheld. HSHS St. Joseph's Hospital offers patients and their families spiritual and emotional support and spiritual counseling 24 hours a day. A chaplain and your own pastor will be called for you and your family as you desire. The HSHS St. Joseph's Hospital Chapel is located on the first floor. It is available for private visits and prayer. Holy Communion is distributed upon request for those who wish to receive communion.
